Birth, Death or Marriage Certificates

How do I get a birth, death or marriage certificate?

1. For Birth or Death Certificates please fill out one of the following:
• Birth, Death & Marriage Certificates
• Marriage Intentions Form
• Marriage License - How to Obtain
2. Certified copies for legal purposes are $15.00 for the first copy and $6.00 for any additional copies purchased at the same time.  Payment is required before records can be released.
3. Make all checks payable to "City of Old Town."  For those applying by mail, please send photocopies only of proof of identity.  Please do not mail originals.
4. Send payment to:

5. Make sure you have your return address with your request. Your request will be mailed to you the same day it is received in the Clerk's office.
